Medical Care Provider

A Medical Care Provider at Abode Care Partners delivers comprehensive, evidence-based medical care to seniors and individuals with complex needs in the comfort of their own homes or assisted living facilities.
The ideal candidate will possess a Master's degree in Nursing and be licensed as an Active state Nurse Practitioner. With a strong knowledge base of HIPAA and OSHA regulations, they will work collaboratively with caregivers and healthcare professionals to maintain up-to-date patient records and ensure seamless care transitions.
This role offers a competitive annual salary range of $105,000 to $120,000, based on performance, with opportunities for professional growth and a flexible schedule.
Responsibilities- Providing Evidence-Based Care
- Develop, implement, and evaluate comprehensive plans of care tailored to each patient's unique needs.
- Conduct thorough examinations, diagnose chronic and urgent conditions, and provide effective treatment strategies.
- Order and interpret laboratory tests and diagnostic imaging as necessary.
- Collaboration and Communication
- Work closely with caregivers, family members, and other healthcare professionals to ensure cohesive care.
- Maintain accurate, detailed patient records, incorporating all relevant health information.
- Patient Referrals and Support
- Identify situations requiring specialist input and facilitate referrals to optimize patient outcomes.
- Nursing Expertise
- Current, active state Nurse Practitioner licensure.
- Master's degree in Nursing (MSN).
- Interpersonal and Organizational Skills
- Demonstrated proficiency in interpersonal communication, both written and verbal.
- Strong time management skills, with a focus on accountability for daily workflow.
