Care Management Assistant Job Summary

The Care Management Assistant plays a vital role in supporting the licensed social worker or nurse case manager with transition planning activities at Johns Hopkins Medicine. This position requires excellent communication and organizational skills to work directly with patients, families, and vendors to set up services for implementing the plan of care.

Key Responsibilities:

Collaborate with social workers and case managers to facilitate appropriate placement in rehabilitation facilities or alternate levels of care.
Develop working knowledge of services offered by various facilities and meet with patients and families to discuss recommendations for rehabilitation facilities, offering choice and entering referrals into the Navihealth application.
Obtain and confirm payment or authorization information, confirm acceptance to facilities, and communicate this to patients, families, and teams.
Consult with social workers and case managers for any barriers or issues related to facility choice, facility availability of beds, or other issues to resolve the issue and ensure a smooth transition for patients.

Requirements:

High School Diploma required; an associate's degree in health science or a closely related field is preferred.
A minimum of one year of previous hospital or long-term care experience, working in case management and admissions.
