Key Responsibilities:
  • Patient Education: Educate patients on PD (CAPD /CCPD) and home dialysis, ensuring they understand the treatment process and can manage their care effectively.
  • Coordination of Care: Collaborate with multidisciplinary team members to coordinate patient care at home, ensuring seamless transitions and optimal outcomes.
  • Patient Evaluation and Modality Options: Meet and evaluate new patients, explaining modality options and home dialysis requirements to ensure they receive the best possible care.
  • Liaison and Communication: Serve as a liaison between patients, supply companies, primary care physicians, and transplant teams to ensure continuity of care and address any concerns or issues.
  • Equipment and Water Quality: Develop a thorough understanding of dialysis equipment and water quality in the home, ensuring patients receive safe and effective treatment.
  • Care Plan Meetings and Charting: Participate in care plan meetings, accurately documenting patient information and maintaining complete and accurate records.
  • Patient Interaction and Communication: Interact with patients and their families to provide instruction, assess daily patient care needs, and communicate patient needs to direct supervisors and physicians.
  • Office Visits and Order Implementation: Participate in patient office visits with physicians, accurately receiving and implementing new orders.
  • Test Scheduling and Results Communication: Schedule, obtain, review, and communicate test results to physicians, relaying order changes to patients and caregivers.
  • Record Keeping and Reporting: Complete required clinic records, including occurrence reports, CQI projects, and primary nurse duties, as well as daily computer entry and reporting requirements.
  • Education and Professional Development: Complete and attend required educational programs to stay up-to-date on the latest best practices and industry developments.
Qualifications and Requirements:
  • Education: Graduated from an accredited Registered Nurse program with a current Alaska license or ability to obtain.
  • Experience: 1 year of nursing experience required, with dialysis experience preferred.
  • Certifications: BCLS and ACLS certification preferred.
  • Leadership and Organizational Skills: Demonstrated leadership ability and organizational skills to manage multiple responsibilities and priorities.
  • Computer Software Knowledge: Proficient in computer software and able to navigate electronic medical records and other relevant systems.
  • Communication and Interpersonal Skills: Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
  • Government Standards and Compliance: Ability to understand and implement government standards and regulations related to home dialysis care.
Physical Job Requirements:
  • Standing, Bending, and Walking: Frequent long periods of standing, bending, and walking, with the ability to lift up to 30 lbs.
  • Travel and Flexibility: Occasional travel by airplane may be required, with flexibility to travel between clinics and patients' homes.
