About the Role:

We are seeking a skilled Healthcare Coordinator Specialist to join our team at Insight Global. The successful candidate will be responsible for supporting two different groups within our organization: Transition of Care and General Care Management.

Key Responsibilities:

  • Assist with the transition of patients after discharge, ensuring a smooth transition and follow-up appointments
  • Identify the most suitable group within our organization for each patient and facilitate their transfer
  • Utilize strong critical thinking skills and excellent communication to effectively manage patient care

Requirements:

  • 2+ years of experience in a healthcare-related coordinator role
  • Good understanding of medical terminology
  • Strong critical thinking skills and ability to work with healthcare systems
