At UnitedHealth Group, we're redefining the future of healthcare by providing innovative and personalized care to our patients. As a Senior Nurse Practitioner in our Home and Community Care team, you'll play a vital role in delivering high-quality, patient-centered care to those who need it most.

With a focus on gerontology, you'll work closely with our interdisciplinary care team to coordinate the delivery of clinical care to our highest-risk health plan members. Your expertise will be invaluable in managing complex medical situations, conducting advanced illness and advanced care planning conversations, and providing patients and caregivers with counseling and education.

Key Responsibilities:

  • Conduct comprehensive assessments and develop personalized care plans
  • Manage members through acute medical situations or exacerbations of chronic conditions
  • Ensure accurate and complete documentation, including ICD-10 conditions
  • Communicate and collaborate with the interdisciplinary care team
  • Provide patients and caregivers with counseling and education

Requirements:

  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted license in the state where you reside, or ability to obtain by start date
  • Active Nurse Practitioner certification through a national board
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
  • Ability to gain a collaborative practice agreement, if applicable in your state
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car

Preferred Qualifications:

  • 1+ years of clinical experience in practice, preferably in a long-term care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
  • Understanding of geriatrics and chronic illness
  • Understanding of advanced illness and end-of-life discussions
