Rehabilitation Social Worker II

We are seeking a highly skilled Rehabilitation Social Worker II to join our team at Hope Network Rehabilitation Services.
About the Role:
The Rehabilitation Social Worker II will be responsible for conducting psychosocial assessments with individuals and families, leading the interdisciplinary team in assessing the family's ability to understand and cope with the consumer's challenges, and providing assurance to the family that the team is working towards the consumer's goals.
Key Responsibilities:
- Conduct psychosocial assessments with individuals and families and document this information in the Social Work evaluation.
- Lead the interdisciplinary team in assessing the family's ability to understand and cope with the consumer's challenges.
- Provide assurance to the family that the team is working towards the consumer's goals.
- Responsible for identifying a clear picture of the patient's pre-injury lifestyle, abilities, skill sets, and interests to enable the team to best approach the patient in a manner that is meaningful to the patient and the family.
- As the primary discharge planner, ensure that individuals on caseload have a smooth transition as they discharge from our program to home or another level of care.
- When assigned, serve as a primary therapist to provide and/or coordinate psycho-emotional services to individuals.
- Is the primary family contact person.
- Facilitates clinical reviews and team meetings as applicable/assigned.
- Function as case manager when assigned to coordinate both internal and external services provided to individuals and families.
- Assist with annual revisions/updates to the Family Resource Notebook.
- Encourages good communication with internal and external disciplines/persons. Portrays a positive professional image. Communicates appropriate information to supervisor.
Requirements:
- Master's degree in Social Work (M.S.W.) from an accredited college or university.
- Licensed as a Social Worker by the State of Michigan.
- Experience in a health/human service field with an emphasis in rehabilitation and discharge planning.
