About the Role

Mederi Caretenders is seeking a skilled Speech Therapist to join our team in providing high-quality home health services to our patients. As a Speech Therapist, you will be responsible for assessing and evaluating patient care needs related to treating speech and language disorders, and functional training in communication, swallowing, and cognitive impairments.

Key Responsibilities
  • Assessment and Evaluation: Conduct thorough assessments and evaluations of patient care needs, using standardized and/or criterion-referenced tools, to determine the most effective treatment plan.
  • Treatment Planning: Develop and implement individualized treatment plans, in accordance with the plan of care, to address patient's speech and language disorders.
  • Interventions and Progress Monitoring: Provide evidence-based interventions aimed at improving and enhancing patient's well-being, and regularly evaluate patient's progress to adjust treatment plans as needed.
  • Communication and Collaboration: Communicate effectively with patients, families, caregivers, and other healthcare team members to ensure seamless care coordination and patient-centered care.
  • Documentation and Reporting: Maintain accurate and timely documentation of patient's progress, treatment plans, and outcomes, as required by regulatory and company standards.
Requirements
  • Licensure: Current licensure in Speech Therapy in the state of FL.
  • Certifications: Current CPR certification.
  • Education and Experience: Bachelor's degree in Speech-Language Pathology or related field, and relevant experience in home health or related settings.
