Job Description

As a Nurse Care Specialist, you will play a vital role in improving the health and well-being of our clients. Your responsibilities will include:

  • Health Management: Provide nursing care across various settings, including home health and community-based programs.
  • Collaboration & Communication: Work closely with interdisciplinary teams to address medical concerns and develop treatment plans.
  • Training & Oversight: Provide training and guidance to direct support staff on health care and hygiene practices.
  • Client Support: Assist clients in coordinating medical appointments and treatments, and conduct home visits to monitor their health.
Required Skills and Qualifications

To be successful in this role, you will need:

  • Graduation from an accredited school of nursing and registration and licensure (RN) to practice nursing in the state of Texas.
  • At least one year of experience in Home Health or Public Health, preferably with Intellectual/Developmental Disabilities.
  • A valid Texas Drivers' License and a clear driving record.
