Job Summary

The Clinical Care Manager is responsible for coordinating screening and providing intervention to patients with complex chronic care needs. This role serves as a connector between patient needs and resources provided by the care management program, supporting whole health outcomes and communicating progress to healthcare organizations.

Key Responsibilities
  • Provide consultation and academic support to residents and interdisciplinary teams for care coordination.
  • Offer comprehensive consultation on disease management assessment and mental/behavioral health treatment options to established patients.
  • Assess patients' and/or families' bio-psycho-social situations, document data, and develop individualized care plans.
  • Review and update care plans based on risk-determined calendar cycles.
  • Maintain a full case load as defined by manager and program requirements, enrolling new patients in a timely fashion.
  • Identify and follow-up on referrals to ensure continuity of care and patient/family needs are met.
  • Complete disease-specific education and thorough assessments with patients and families, maintaining communication with providers and care teams.
  • Perform crisis intervention, individual counseling, family counseling, advocacy, and telephone triage for patient health concerns.
Requirements
  • Graduate of an accredited school of nursing with a Bachelor's degree in a related field.
  • 2-3 years of clinical experience and 2-3 years of EMR experience preferred.
  • Constant communication with patients, families, and healthcare providers, exchanging accurate information.
  • Demonstrated skills in the designated and certified clinical area of practice and ability to work collaboratively on a healthcare team.
  • Ability to effectively and efficiently handle a demanding workload involving multiple tasks.
  • Proficient in MS Office Business Applications, including Outlook, Word, PowerPoint, and Excel.
Certifications/Licenses
  • Current and valid CPR certification.
  • Active and in good standing State of Illinois RN license required.
Physical Demands
  • Ability to remain in a stationary position 50% of the time and move around the clinic site 50% of the time.
  • Constantly operates a computer, computer printer, copy machine, and telephone.
  • Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
Other Skills
  • Bilingual (required): Ability to read, write, and speak proficiently using both English and Spanish language.
  • Constantly communicates with other healthcare providers, exchanging accurate information.
  • Follow-through, assume responsibility, and use good judgment.
  • Maintain professionalism under stressful situations.
  • Self-motivated and directed with the ability to prioritize and work efficiently under pressure.
  • Ability to understand and follow verbal and written communication.
  • Detail-oriented with the ability to work with minimal/no supervision.
  • Willingness to be part of a team-unit and cooperate in the accomplishment of departmental goals and objectives.
  • Effective and creative problem-solving.
