Clinical Nurse Coordinator

We are seeking a highly skilled and compassionate Clinical Nurse Coordinator to join our team at HealthEcareers - Client. As a Clinical Nurse Coordinator, you will play a vital role in providing exceptional patient care and leading our dialysis team.
About the Role:- Lead a team of nurses and healthcare professionals to deliver high-quality patient care in a dialysis setting.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Monitor patient progress, identify areas for improvement, and implement changes to optimize care outcomes.
- Provide education and support to patients and their families to promote self-care and independence.
- Current Registered Nurse (RN) license in the state of practice.
- At least 18 months of registered nursing experience, preferably in dialysis or a related field.
- Strong leadership and communication skills, with the ability to motivate and inspire a team.
- Excellent clinical skills, with the ability to assess patient needs and develop effective care plans.
- Basic computer skills and proficiency in MS Word and Outlook.
