Position Summary:

The Lead Case Manager will operate under the guidance of the Enhanced Care Management (ECM) Supervisor, delivering essential support to patients eligible for ECM services. This role is pivotal within the interdisciplinary Case Management team, managing a personal caseload of approximately 30-60 patients based on various factors such as location and patient needs.

Key Responsibilities:
  1. Conduct outreach and engage with patients, assessing their eligibility for the Enhanced Care Management program.
  2. Develop personalized care management plans that address the physical, mental, and social health needs of patients.
  3. Empower patients to actively participate in their care through coaching and education on self-management strategies.
  4. Coordinate care activities in alignment with the established care management plan, ensuring seamless communication among the multidisciplinary team.
  5. Maintain regular contact with patients and their support systems to facilitate ongoing care coordination.
  6. Assist patients in navigating community resources and benefits to support their health and well-being.
  7. Document patient interactions and progress accurately in the electronic health record (EHR).
  8. Support the transition of patients from higher levels of care, ensuring continuity and adherence to treatment plans.
Qualifications:
  • A bachelor's degree in social work, psychology, or a related field, or a high school diploma with extensive experience in case management.
  • Experience in community outreach and resource linkage is preferred.
  • Strong knowledge of local healthcare and social service resources is essential.
  • Proficient computer skills, including familiarity with healthcare systems.
  • Bilingual capabilities in Spanish are advantageous.
  • Valid driver's license and ability to travel as needed.
Physical Requirements:
  • Ability to perform tasks involving standing, walking, sitting, typing, and lifting up to 25 pounds.
