Geriatric Nurse Practitioner

The Personalized Health Partners (PHP) Nurse Practitioner is responsible for delivering high-quality primary care in a personalized, patient-centered model. This role involves diagnosing, treating, and managing acute and chronic conditions through wellness visits, early detection, and timely treatment. The Nurse Practitioner collaborates with primary care physicians and interdisciplinary teams to manage patient care and involves residents and their families in developing care plans.
Key Responsibilities:- Assess, diagnose, and manage chronic and acute conditions.
- Provide health education and support to patients and their families.
- Develop and implement treatment plans, communicating effectively with patients and other healthcare providers.
- Order and interpret lab and diagnostic tests, adjusting treatment plans as needed.
- Maintain accurate and compliant documentation within our electronic medical record system.
- Collaborate with local and regional operations and clinical leadership to achieve organizational goals.
- Support strategic initiatives related to value-based care and patient satisfaction.
- Practice ethically and in accordance with professional standards and board certification.
- Comply with state and federal regulations, guidelines, and laws.
- Master's degree in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- National certification as an AANP or ANCC.
- Active, unrestricted state license.
- Active DEA license.
- US work authorization.
- At least 2 years of clinical experience as a Nurse or Nurse Practitioner in geriatric, adult, or family practice settings.
- Ability to work in a flexible environment and schedule.
- Passion for delivering healthcare to seniors and low-income populations.
- Training or experience in geriatrics.
- At least 3 years of clinical experience as a Nurse Practitioner.
- Experience with coding, ICD-10, and HCC (Hierarchical Condition Categories).
- Experience or training in value-based care models.
- Training or experience in geriatric, assisted living, and/or skilled nursing settings.
- Experience working in a collaborative healthcare setting to drive positive outcomes and achieve goals.
