Oak Street Health is seeking a highly skilled and compassionate Primary Care Nurse Practitioner to join our team. As a key member of our Care Team, you will provide exceptional primary care to our patients, working collaboratively with our doctors and other healthcare professionals to deliver high-quality, patient-centered care.
Responsibilities- Provide routine and acute care to patients, including office visits, annual wellness visits, and health risk assessments.
- Care coordination with other providers, specialists, testing facilities, and agencies.
- Population health leadership, including making sure all eligible patients receive evidence-based screenings and preventive care.
- Phone triage and outreach to patients.
- Patient education and support.
- Oak Street Health promotional activities.
- Home visits as needed.
- Experience in primary care, internal medicine, geriatrics, or family medicine.
- Experience with risk adjustments and/or home assessments.
- Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
- Active, non-probationary state NP license.
- National certification as a Nurse Practitioner through AANP or ANCC.
- Genuine passion for primary care and a commitment to delivering exceptional patient care.
- Intrinsically motivated and able to work collaboratively as part of a team.
- Spanish language skills are strongly preferred, depending on clinic location.
