Position Overview

We are seeking a dedicated Certified Nurse Practitioner specializing in Adult, Geriatric, or Family care to provide essential in-house services within a Subacute, Skilled, and Long-Term Care environment.

Key Responsibilities

The selected candidate will:

  • Participate in daily morning meetings to discuss patient care.
  • Conduct rounds to address acute medical issues and follow up on diagnostic tests.
  • Assist nursing staff in identifying and managing acute illnesses to minimize unnecessary hospital transfers.
  • Engage in facility initiatives such as antibiotic stewardship and Quality Assurance Performance Improvement (QAPI).
  • Contribute to nursing education and training programs.
  • Collaborate with physicians and other Nurse Practitioners to coordinate patient care.
Qualifications

The ideal candidate will possess:

  • Experience in long-term care or with older adult populations.
  • Skills in managing multiple chronic conditions.
  • Interest or experience in psychiatric patient care is advantageous.
