Thrive Skilled Pediatric Care LLC is seeking a dedicated and compassionate Pediatric Home Health Nurse to provide high-quality clinical home care to medically fragile children in El Campo, TX.
About the Role:
- Provide skilled pediatric home care to children with complex medical needs
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Conduct thorough assessments, develop and implement care plans, and provide ongoing care and support to children and their families
- Communicate effectively with families, caregivers, and other healthcare professionals to ensure seamless care coordination
- Participate in ongoing education and training to stay current on best practices and evidence-based care
Requirements:
- RN/LVN with a current valid license to practice as issued by the State Board of Nursing
- Current CPR certification
