Case Management Department Head

As a key member of our healthcare team, the Case Management Department Manager plays a vital role in ensuring the highest quality patient care. This leader is responsible for managing the daily clinical, financial, and quality performance of our Case Management Team, driving change management for performance improvement, and supporting the case management teams throughout the hospital.
Responsibilities- Assess patient needs and coordinate, monitor, evaluate, and advocate for multiple services to meet the specific patient's complex needs.
- Collaborate with the multidisciplinary team to support care progression and plan for a safe, appropriate discharge.
- Support case management processes and drive change management for performance improvement.
- Lead and develop the Case Management Team to achieve exceptional patient outcomes.
- Active Registered Nurse (RN) in Indiana or a compact licensed state.
- A minimum of three (3) years of acute hospital care experience.
- Prior leadership experience.
- BSN degree.
- Case Management Certification.
- Recent clinical experience in specific areas required for specialty clinical groups or clusters.
