Inpatient Rehabilitation Facility Coordinator
Aiken Regional Medical Centers, a premier healthcare provider, is dedicated to delivering exceptional medical services to the community. This role focuses on the comprehensive management, execution, and submission of the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI) in collaboration with interdisciplinary teams, ensuring accuracy and adherence to timelines. The position also involves educating staff on the application of Care Tool completion and documentation for GG coding, as well as assisting in data collection and performance improvement analysis.
Core Duties:
Effectively manage multiple projects with prioritization skills.Exhibit strong time management capabilities.Utilize Excel proficiently, including creating tables, basic formulas, and graphs.Understand and operate the UDS data management system.Demonstrate competence in GG coding using Care Tool.Apply pressure ulcer staging knowledge to IRF-PAI documentation.Establish and maintain systems for data collection related to IRF-PAI and performance improvement initiatives.Attend relevant hospital committee meetings as designated by the Program Director.Serve as a resource for the Rehabilitation unit and hospital staff.Complete all mandatory competencies for the hospital and rehabilitation department within specified timeframes.Participate in rehabilitation unit meetings and in-services.Gather and analyze data from Medical Records for IRF-PAI completion as per Medicare requirements.Review and verify IRF-PAI submissions for accuracy before finalization.Coordinate with various hospital departments to ensure timely billing and proper retention of IRF-PAI documentation.Facilitate annual and ongoing training for therapy and nursing staff, including competency assessments through UDS.Identify and rectify discrepancies in Care Tool documentation in real-time.Report non-compliance issues to the Program Director regarding interdisciplinary team members.Assist the Rehab Director and Medical Director with outcome reporting and trend monitoring in patient care.Support the management of performance improvement processes and data collection.Maintain professionalism and ethical standards in all communications with staff.Qualifications
Educational Background: An Associate's Degree in Nursing is required; a Bachelor of Science in Nursing (BSN) is preferred.
Experience: A minimum of 2 years in a clinical or hospital environment with a solid understanding of clinical conditions. Experience in rehabilitation or skilled nursing settings is advantageous.
Licenses and Certifications: A current RN license in South Carolina is mandatory. Must pass the Quality Improvement credentialing exam bi-annually.
Skills and Competencies: Excellent written and verbal communication skills are essential. The ability to work independently, along with strong critical thinking and organizational skills, is required. Proficiency in computer applications, including word processing, spreadsheets, and databases, is necessary.
