About the Role

We are seeking a highly skilled Clinical Coordinator to join our team at Tufts Medicine Care at Home. As a Clinical Coordinator, you will play a vital role in the coordination of clinical services, working closely with our clinical staff to provide high-quality care to patients and families in their homes.

Key Responsibilities
  • Triage calls related to patient care, including physician orders, staff issues, and other clinical issues.
  • Provide effective coordination of clinical services, consistent with the objectives of the agency.
  • Work closely with clinical staff to provide necessary services for patients and families in a timely manner, including the provision of direct patient care.
  • Communicate regularly with Clinical Managers and the Clinical Director on issues related to clinical practice or performance.
  • Assist with individual caseload reviews or other audits as assigned by the manager or Clinical Director.
  • Assist with orientation and preceptor of clinical staff.
  • Evaluate performance of each individual in relation to established standards in collaboration with the clinical manager.
Requirements
  • Registered Nurse (RN) with three (3) years of community health experience or a combination of at least one year of acute medical/surgical nursing experience and 2 years of home health experience.
