About Our Hospice Care Team

We are seeking a skilled Hospice Intake Coordinator to join our team at Trinity Hospice.

Job Summary:

The primary function of the Hospice Intake Coordinator role is to facilitate the referral intake process, coordinate care with the interdisciplinary team and the patient and their family, and the referral source.

Key Responsibilities:
  • Receive and document new referrals in a timely manner
  • Verify Medicare, Medicaid, and private insurance benefits promptly
  • Evaluate physician orders, H&Ps, and transfer documentation for completeness and accuracy
  • Create patient charts in EMR software (Consolo)
  • Notify the clinical team of pending admissions
  • Ensure all relevant documents are uploaded to patient charts
  • Maintain accurate referral logs and generate daily reports
  • Verify benefits for private insurance patients on a monthly basis
  • Assist in ordering equipment as directed by DONs or PCMs
  • Attend weekly and monthly meetings with marketing and staff teams
  • Promote effective communication among patients, staff, healthcare professionals, and referral sources
  • Participate in our organization's QAPI program

We are looking for an individual with a strong foundation in healthcare management and excellent interpersonal skills. A High School Diploma with a Bachelor's Degree in Healthcare Management (or equivalent) is preferred. Relevant work experience in the healthcare industry, computer skills using Google Suite products, and knowledge of medical terminology are also desired.
