Role of the Placement Coordinator:

The Placement Coordinator operates under the guidance of the Admission/Placement Coordinator, executing consultative services and technical assistance for individuals transitioning from State Supported Living Centers to community environments.

Responsibilities include overseeing the Community Living Discharge Planning (CLDP) process and formulating transition plans.

The Coordinator serves as a vital resource for interdisciplinary team members, community providers, and Local Authority staff, offering expertise in community transition methodologies.

Duties encompass assisting with admissions, transfers, discharges, and additional assigned responsibilities, with opportunities to train others. This role operates under moderate supervision, allowing for some initiative and independent judgment.


Key Responsibilities:

- Maintain regular attendance and may be required to adhere to specific shift schedules, including potential rotating shifts and overtime as per agency policies.

- Collaborate with Qualified Intellectual Disability Professionals and interdisciplinary team members to identify necessary supports and services for individuals referred for community transition.

- Provide specialized knowledge regarding community transition, acting as a liaison to staff, community organizations, and the public to clarify program specifics and requirements.

- Participate in interdisciplinary planning meetings concerning community referrals and transition planning.

- Ensure timely and accurate completion of required documentation within established timelines.

- Assist in scheduling and ensuring that necessary in-services are conducted for potential providers before individuals visit community homes for trial transitions.

- Review discipline transition assessments (medical, behavioral health, dental, vocational, etc.) and draft transition plans for distribution to relevant parties in preparation for CLDP meetings.

- Collaborate with various disciplines to resolve inconsistencies or issues identified in assessments.

- Schedule, facilitate, and support the planning and development of CLDPs, identifying community resources for individuals transitioning from SSLCs to community settings.

- Ensure the completion and distribution of the CLDP to individuals, their legal representatives, community providers, and Local Authorities prior to the transition.

- May assist in the development of policy and procedure manuals and contribute to administrative reports, studies, and specialized projects.


- Additional duties may include participating in disaster response and recovery efforts or Continuity of Operations (COOP) activation, which may necessitate alternate shift patterns or locations.


Required Skills and Abilities:

- Proficient in report preparation and effective oral and written communication.

- Ability to engage professionally with individuals with intellectual disabilities, their families, staff, community providers, and others.

- Knowledge of DADS community programs, including ICF/ID and waiver programs.

- Capable of gathering, assembling, correlating, and analyzing information to devise solutions to challenges.

- Proficient in Microsoft Office Suite, including WORD and EXCEL.

- Ability to manage multiple responsibilities and effectively plan work activities.
