**Role Summary:** The Placement Coordinator operates under the guidance of the Admission/Placement Coordinator.

This role involves providing consultative services and technical assistance for individuals transitioning from State Supported Living Centers to community environments.

The Placement Coordinator is responsible for managing the Community Living Discharge Planning (CLDP) process and formulating transition plans.

This position serves as a vital resource for interdisciplinary team members, community providers, and Local Authority staff, offering expertise in community transition processes.

Responsibilities include assisting with admissions, transfers, discharges, and other related tasks, with potential training of new staff.


**Key Responsibilities:**

  1. Maintain regular attendance and may be required to work specific or rotating shifts, including extended hours as per agency policy.
  2. Consult with Qualified Intellectual Disability Professionals and interdisciplinary team members to identify necessary supports and services for community transition.
  3. Provide expertise in community transition, acting as a liaison to staff and community organizations to clarify program specifics and requirements.
  4. Participate in interdisciplinary team planning meetings related to community referrals and transition planning.
  5. Ensure timely completion of required documentation within established timelines.
  6. Assist in scheduling and facilitating in-services for potential providers before individuals visit community homes.
  7. Review transition assessments and draft transition plans for distribution to relevant parties.
  8. Collaborate with various disciplines to resolve inconsistencies identified in assessments.
  9. Schedule and support the planning and development of CLDP, identifying community resources for individuals transitioning to community settings.
  10. Ensure completion and distribution of the CLDP prior to community transition.
  11. Assist in the development of policy manuals and preparation of administrative reports and specialized projects.
  12. Perform related duties as assigned, including participation in disaster response and recovery efforts as needed.

**Required Skills and Abilities:**

  • Proficient in preparing reports and communicating effectively both orally and in writing.
  • Ability to engage professionally with individuals with intellectual disabilities, their families, staff, and community providers.
  • Knowledge of community programs, including ICF/ID and waiver programs.
  • Strong analytical skills to gather, correlate, and assess information to develop solutions.
  • Competence in Microsoft Office applications, including WORD and EXCEL.
  • Ability to manage multiple responsibilities and effectively plan work activities.
