Job SummaryWe are seeking a highly skilled Field Nurse Case Manager to join our team in the Bozeman or Billings, MT area. As a Field Case Manager, you will work in a home-based setting, traveling to various locations to provide medical case management services for work-related injuries. Your primary responsibilities will include assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals, working closely with insurance carriers, medical care providers, attorneys, employers, and employees.
Key Responsibilities- Conduct thorough assessments of injured workers to develop comprehensive case management plans
- Collaborate with healthcare providers, insurance carriers, and other stakeholders to ensure seamless care coordination
- Monitor progress and report results to employers and insurance carriers
- Provide education and support to injured workers and their families
- Stay up-to-date with industry developments and best practices in case management
- Current, unencumbered compact Registered Nurse (RN) license in Montana
- Experience in case management of workers' compensation cases preferred
- Recommended certifications (CCM, CRRN, CDMS, CRC) preferred
- Strong organizational and communication skills
- Ability to work autonomously with minimal oversight
- Valid driver's license and good driving history
