Licensed Clinical Social Worker

About the Role:

We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Rehab Without Walls Neuro Rehabilitation. As a Licensed Clinical Social Worker, you will work with patients who have experienced a traumatic brain injury, spinal cord injury, or stroke/CVA, providing comprehensive evaluations and developing measurable objectives to help them achieve their personal, career, and independent living goals.

Responsibilities:
  • Perform comprehensive evaluations, assessing the social, emotional, and support needs of patients and their families
  • Set measurable objectives in conjunction with the rehab team
  • Assist physicians and other health team members in understanding significant social and emotional factors related to patient health problems
  • Instruct and counsel patients and families in treating and coping with social and emotional responses connected with illnesses
  • Participate in care coordination activities and act as a resource to other health team members in identifying and resolving patient needs
  • Identify and assist patients and families in utilizing appropriate community resources to achieve measurable objectives
Qualifications:
  • Master's Degree in Clinical Social Work (LCSW)
  • Current Licensed Clinical Social Worker licensure in the state of practice
  • Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury, or acute diagnosis
  • CPR certification required
  • Ability to lift 50 pounds
  • Moving, lifting, or transferring of patients may involve lifting up to 100 pounds following safety procedures
  • Duties require fine motor skills, visual acuity, and walking/standing for extended periods
  • Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently
