About the Role

The Home Health RN Clinician 2 is a seasoned clinician responsible for delivering high-quality patient care in the comfort of their own homes. As part of our interdisciplinary team, you will oversee the care planning and coordination for a diverse caseload of patients, working collaboratively with patients, their family physicians, and other healthcare professionals to ensure seamless care delivery.

Key Responsibilities
  • Provide effective direct care to patients in their homes, utilizing your expertise to develop and implement personalized treatment plans.
  • Collaborate with the healthcare team to ensure comprehensive care coordination, addressing the unique needs of each patient.
  • Manage complex patient cases, applying your refined clinical skills to optimize patient outcomes.
  • Participate in performance improvement activities, contributing to the advancement of our clinical practices and standards.
  • Engage in ongoing professional development, staying current with best practices and evidence-based care guidelines.
Requirements
  • Graduate of an accredited nursing program, with a Bachelor of Science in Nursing required within 5 years of hire.
  • At least 1 year of relevant experience in home health care or a related field.
  • Current licensure as a Registered Nurse in the Commonwealth of Virginia, with a valid Virginia Driver's License.
  • American Heart Association (AHA) Health Care Provider BLS certification.
