Key Responsibilities- Provide direct patient care in the home setting, utilizing your expertise to assess, plan, and implement individualized care plans.
- Collaborate with patients, families, and healthcare professionals to ensure seamless care coordination and communication.
- Conduct thorough assessments, identify patient needs, and develop and implement care plans that promote optimal health outcomes.
- Manage complex patient cases, utilizing your expertise to develop and implement treatment plans that meet the unique needs of each patient.
- Participate in performance improvement activities, contributing to the development and implementation of quality improvement initiatives that enhance patient care and outcomes.
- Graduate of an accredited nursing program required; Bachelor of Science in Nursing required within 5 years of hire.
- 1 year of relevant experience in home health care or a related field.
- Licensed to practice as a Registered Nurse in the Commonwealth of Virginia required; valid Virginia Driver's License required.
- American Heart Association (AHA) Health Care Provider BLS certification required.
