About the Role

CareATC is seeking an experienced RN Case Manager to join our team. As a key member of our Total Health Solutions team, you will provide platinum-level patient care and excellent customer service.

Key Responsibilities
  • Conduct comprehensive assessments to identify patient needs and develop personalized care plans.
  • Provide clinical education on risk prevention and medication management to patients and their families.
  • Evaluate patient compliance with care plans and medication regimens.
  • Act as a patient advocate, helping them make informed decisions about their treatment options.
  • Collaborate with healthcare providers, health center staff, and other stakeholders to ensure seamless care coordination.
Requirements
  • Bachelor's degree in Nursing from an accredited university.
  • Current RN licensure and multi-state licensure required.
  • Minimum 5 years of experience in a healthcare setting, with knowledge of health insurance plans, electronic medical records, and Microsoft Office platforms.
  • CPR certification and current health records with appropriate immunizations.
