**Key Responsibilities:**

* Provide social interaction and reassurance to patients and families
* Assist patients with daily living activities, such as personal hygiene and physical activity
* Report changes in patient condition or environment to the attending nurse
* Support patient teaching and other procedures based on patient plans of care
* Prepare and submit documentation of observations, findings, and services provided
* Administer emergency first aid when needed

**Qualifications:**

* Valid CNA certification required
* Valid driver's license, auto insurance, and transportation in order to travel to patient locations
* Verbal and written communication skills
* Ability to complete patient charting on a daily basis
