Job Summary

We are seeking a Certified Nursing Assistant (CNA) to join our team of hospice care professionals. As a CNA, you will play a vital role in providing personalized care and support to our patients in their place of residence. Your primary function will be to assist with daily living activities, provide emotional support, and work collaboratively with our interdisciplinary team to ensure the highest quality of care.

Responsibilities
  • Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming
  • Assist with medication management and administration
  • Monitor and report changes in patient condition to the supervising nurse
  • Participate in patient and family education and support
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans
Requirements
  • High School Diploma or equivalent
  • Current CNA certification
  • At least one year of experience as a CNA in a hospice or palliative care setting
  • Reliable transportation and valid driver's license
