Nurse Care Coordinator

We are seeking a skilled Licensed Practical Nurse to join our team at Avamere Riverpark of Eugene. As a key member of our nursing staff, you will be responsible for providing high-quality patient care and contributing to a positive and supportive work environment.
Responsibilities:
• Complete accurate and timely record keeping in regards to patient admissions, transfers, and discharges.
• Coordinate with nursing staff and support personnel to plan daily services, programs, and activities that meet the needs of our residents.
• Prepare and administer medications, ensuring accuracy and completeness of medication cards.
• Review and revise resident care plans to ensure they align with individual needs and goals.
• Provide leadership and guidance to nursing personnel, ensuring direct care is delivered in accordance with resident care plans and wishes.
• Develop and maintain positive relationships with residents, family members, and other stakeholders involved in patient care.
Requirements:
• Possess a current, unencumbered LPN license in the state of Oregon.
• Demonstrate knowledge of nursing and medical practices, as well as laws and regulations governing nursing care facilities.
• Experience with Electronic Medical Records and computer documentation systems.
• Maintain confidentiality of patient information in accordance with HIPAA guidelines.
