Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Kaiser Permanente. As a Physical Therapist, you will be responsible for the physical therapy management of patients, working closely with our Director, Assistant Director, and Clinical Supervisor to ensure the highest level of care.
Key Responsibilities- Evaluate patients and interpret examination findings to determine the nature and extent of dysfunction relative to persons limited by medical problems, birth defects, or health-related conditions.
- Perform clinical testing to determine muscle strength, reflexes, sensation, coordination, range of motion, and other sensory motor skills.
- Establish a plan of care to achieve established goals relative to age, medical history, and dysfunction.
- Administer or prescribe modalities and other physical agents.
- Administer or prescribe exercise programs/development of patient care programs.
- Demonstrate professional competence and clinical reasoning in assessing patients' conditions at the onset and through all phases of skilled physical therapy.
- Routinely take usual and special precautions relative to the age, medical history, and condition of the patient, and the type of treatment being given.
- Regularly reassess clinical signs and symptoms to determine the effectiveness of treatment, progression towards goals, and the need for modification of treatment and/or goals.
- Implement the treatment program through direct treatment of the patient.
- May delegate a portion of the treatment plan to appropriate supportive personnel.
- Coordinates the treatment plan and participates in discharge planning with other healthcare personnel as appropriate.
- May participate in relevant patient care rounds and case management conferences.
- Provides advice and education to patients and families.
- Discharges patients from physical therapy services when skilled therapy is no longer medically necessary and when clinically appropriate.
- Completion of an accredited PT training program with a Bachelor's, Master's, or Doctorate degree in Physical Therapy.
- Physical Therapist License - Applicant (California) required at hire OR Physical Therapist License (California) required at hire.
- Basic Life Support required at hire.
- National Provider Identifier required at hire.
- Member APTA.
- Peds experience preferred.
This position will work in IP & OP settings. The primary location will be WT. The position may work at other RSA locations based on operation needs.
The typical schedule will be Wednesday-Friday 8:30am-7pm, Saturday 7:30am-6pm, 8-hour default Monday, Wednesday-Friday 10:30am-7pm, Saturday 9:30am-6pm, including rotating weekends and holidays in seniority section.
The primary location is California, Wildomar, Wildomar Trail Medical Offices.
