Licensed Physical Therapist

Accountable for the management of physical therapy services for patients. Reports to the Director, Assistant Director, and/or Clinical Supervisor.
Key Responsibilities:
Conducts patient evaluations and interprets assessment results to identify the nature and extent of physical impairments related to medical conditions, congenital anomalies, or health-related issues, aiming to restore mobility and functional capabilities in daily activities. Performs clinical assessments to evaluate muscle strength, reflexes, sensation, coordination, range of motion, and other sensory-motor skills while executing clinical analysis and treatment for postural, gait, and activities of daily living (ADL) disorders.Develops individualized care plans to meet established objectives based on age, medical history, and identified dysfunctions.Administers or recommends therapeutic modalities and other physical agents.Designs and implements exercise programs and patient care initiatives.Exhibits professional expertise and clinical judgment in evaluating patient conditions throughout all stages of skilled physical therapy.Consistently observes standard and specific precautions concerning the patient's age, medical history, and treatment type.Regularly reviews clinical indicators to assess treatment effectiveness, progress towards goals, and necessary adjustments to treatment plans and objectives.Delivers treatment directly to patients as part of the therapy program.May assign certain aspects of the treatment plan to qualified support staff.Collaborates on treatment planning and participates in discharge processes with other healthcare professionals as needed.May engage in relevant patient care discussions and case management meetings.Provides guidance and education to patients and their families.Concludes physical therapy services when skilled intervention is no longer medically required and clinically justified.Adheres to Kaiser Permanente's Policies and Procedures, Principles of Responsibilities, and applicable local, state, and federal regulations.Maintains cleanliness in assigned areas and reports any malfunctioning patient care equipment according to departmental maintenance protocols.Ensures accurate and timely documentation of physical therapy services in compliance with state and federal regulations and relevant medical group policies.Collaborates with management to fulfill all mandatory compliance obligations.May require training to address a diverse patient population based on organizational needs.May participate in the clinical training of Physical Therapy Assistants and Aides.Effectively supervises Physical Therapy Assistants and Aides during delegated patient-related and non-patient-related tasks.Demonstrates exemplary internal and external customer service and adheres to the Principles of Responsibility.Basic Qualifications:

Completion of an accredited Physical Therapy training program with a Bachelor's, Master's, or Doctorate degree in Physical Therapy.License, Certification, Registration
Physical Therapist License - Applicant (California) required at hire OR Physical Therapist License (California) required at hireBasic Life Support certification required at hireNational Provider Identifier required at hireAdditional Requirements:
Ability to work within a Labor/Management Partnership environment.If a PT License Applicant (PTLA) or PT Aide Letter recipient: must pass the licensing exam within 120 days from the letter date.Preferred Qualifications:
Membership in the American Physical Therapy Association (APTA).
Notes:
Primary location will be within the designated service area.May work at other locations based on operational needs.Will provide services in both inpatient and outpatient settings.Standard work hours apply.-
