Job Summary

We are seeking a skilled and compassionate Primary Care Clinic Licensed Vocational Nurse I/II to join our Ambulatory Care Enhanced Care Management Team. As a member of our team, you will play a vital role in providing skilled nursing care to patients with complex needs, offering case management and support to help them achieve their health goals.

Key Responsibilities
  • Provide field-based nursing care to patients with complex needs
  • Facilitate care planning, goal setting, and motivational interviews to guide patients through medical, behavioral health, and social services
  • Conduct field-based care, coordinate with providers, and document case notes and services provided in the electronic health record system
  • Develop and maintain relationships with patients, families, and healthcare providers to ensure comprehensive care
  • Participate in quality improvement efforts to enhance patient outcomes and experiences
Requirements
  • Current, valid license as a Vocational Nurse issued by the State of California
  • One year of full-time licensed vocational nursing experience in a healthcare setting
  • Completion of the Primary Care Clinic Licensed Vocational Nurse I competencies established by the Ambulatory Care Nursing Administrator
  • Ability to work effectively in a fast-paced environment and adapt to changing priorities
  • Excellent communication and interpersonal skills
