Job Summary

The Licensed Practical Nurse (LPN) is a vital member of the ElderONE team, providing direct patient care to seniors 55 and older with chronic care needs. As an LPN, you will work collaboratively with a multidisciplinary team to deliver comprehensive medical care and support services, enabling our patients to maintain their independence in the comfort of their own homes.

Responsibilities
  • Provide direct patient care, including administering medication, performing treatment procedures, and providing physical, educational, and emotional support to patients and their families.
  • Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans.
  • Document patient care activities and outcomes in a clear and concise manner, using electronic medical records.
  • Administer medication and give/receive reports as per departmental protocols.

Requirements
  • Graduate from an accredited School of Practical Nursing or equivalent.
  • One year experience working with frail elderly population required.
  • Current licensure in the State of New York.
