Job Summary

The Licensed Practical Nurse (LPN) is a vital member of the ElderONE team, providing direct patient care to seniors 55 and older with chronic care needs. As an LPN, you will work under the direction of a Registered Nurse to deliver high-quality care and support services, allowing our patients to maintain their independence in the comfort of their own homes.

Responsibilities
  • Provide direct patient care, including administering medication, giving/receiving reports, and performing various treatment procedures.
  • Collaborate with physicians and multidisciplinary team members to promote and restore patients' health.
  • Document resident care using computerized medical records.

Requirements
  • Graduate from an accredited School of Practical Nursing or equivalent.
  • One year experience working with frail elderly population required.
  • Current licensure in the State of New York.

Physical Requirements

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.


Licenses/Certifications
LPN - Licensed Practical Nurse - New York State Education Department (NYSED)
BLS - Basic Life Support - American Heart Association (AHA)

Pay Range
$32.50
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
