LPN-Out Patient Care Specialist

As a LPN-Out Patient Care Specialist at Highland Hospital, you will play a vital role in guiding our patients through a seamless process to ensure that their care is of the highest quality and delivered in the most efficient manner. Supervised by a Registered Nurse or Physician, you will assist patients through the appointment and provide supervised nursing services during care. You will make and accept patient phone calls and document the patients symptoms, questions, or concerns. This information will be relayed to the RN or provider for the determination of the action needed. You will participate in care planning discussions, based on RN, MD or NP-determined urgency of care needs. The triage of emergency care situations is the responsibility of the RN or provider and scheduling of acute care appointments to maximize patient care opportunities will be facilitated by you. You will maintain the flow of the provider schedule during patient care sessions minimizing patient wait time whenever possible. You will provide assistance to the physicians and nurse practitioners for patient care follow up needs.

Requirements:
Fulfilled the educational requirements to be an LPN in NYS
Current NYS LPN license and current BLS
3-5 years in OB/GYN experience preferred
