Position Overview

The Pediatric Care Manager II plays a crucial role in supporting members with complex health needs, particularly those with medically fragile children. This position requires a Bachelor's Degree in Nursing and a current, unrestricted RN license in Pennsylvania. The ideal candidate will have a minimum of 3 years of clinical experience as a Registered Nurse, specifically working with pediatric patients in various settings.

Key Responsibilities:

  • Conduct assessments through in-person visits or telephonic interactions to identify care coordination needs.
  • Perform comprehensive evaluations that encompass physical health, mental health, and social determinants of health.
  • Facilitate access to necessary physical, behavioral, and social services.
  • Manage medication protocols, ensuring adherence and conducting regular reconciliations.
  • Identify barriers to care and implement appropriate interventions.
  • Develop individualized care plans aimed at resolving issues and promoting optimal health outcomes.
  • Set measurable goals with defined timelines and communicate these effectively with members and their families.
  • Coordinate with healthcare providers, including primary care physicians and specialists, to ensure comprehensive care.
  • Provide transitional care management and collaborate with care teams to enhance service delivery.

Qualifications:

  • Must possess a valid driver's license and have access to a personal vehicle for member visits.
  • Proficiency in Microsoft Office Suite and electronic medical record systems is essential.
  • 3+ years of experience in case management, particularly in a managed care environment, is preferred.
  • Eligibility for Case Management certification within the first year of employment.
