Key Responsibilities

As a Targeted Case Manager at Aware, you will be responsible for providing case management services to individuals who meet the definition of being developmentally disabled. This will involve:

  • Assessing client needs and developing personalized treatment plans

  • Maintaining accurate records and reporting progress to supervisors

Requirements

To excel in this role, you must possess:

  • A BS/BA Degree in a social work-related field and a minimum of one year of direct work experience in the developmental disability field
  • Strong interpersonal and communication skills, with the ability to work effectively with diverse populations
  • Knowledge of behavioral, clinical, and mental health interventions, and skill-building techniques
  • Ability to maintain confidentiality and handle sensitive information
