About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team at the Kaplan Family Hospice Residence. As a Registered Nurse, you will play a vital role in providing high-quality patient care and supporting our patients and their families during a difficult time.

Responsibilities
  • Establish priorities and organize work and time effectively to ensure seamless patient care.
  • Anticipate and resolve problems, modifying plans and procedures as needed to provide optimal patient outcomes.
  • Assess and assign work assignments to ensure complete utilization of staff and quality patient/family care.
  • Ensure medications and supplies are available for staff to deliver quality patient care.
  • Participate in the evaluation and review of services provided to each patient and family, collaborating with the interdisciplinary team and other agencies/professionals as needed.
  • Provide direct patient care as needed on an emergency basis.
  • Perform accurate initial assessments with timely and accurate admission documentation.
  • Generate initial care plans and perform ongoing assessments of patient and family needs, updating the nursing care plan accordingly.
  • Perform accurate ongoing symptom assessments with appropriate management and follow-through, inclusive of physical, psychosocial, and spiritual needs.
  • Maintain proficiency working with technical equipment, including enteral feeding pumps, suction machines, IV medication pumps, and hydration pumps.
  • Maintain proficiency working with IV access devices, including IV medication administration, IV maintenance, and documentation of same.
  • Administer medication as ordered according to Hospice Residence policy and document accurately.
  • Ensure documentation in the patient's medical record is accurate and complete, reflecting timeliness, accuracy, and a clear clinical picture of the patient.
  • Coordinate patient care services rendered in the Hospice Residence while maintaining standards of care consistent with the Hospice philosophy.
  • Communicate effectively with physicians.
  • Coordinate patient transfer between the Hospice Residence and other departments/facilities.
  • Participate in disciplinary meetings with employees per policy.
  • Function in a leadership role for the staff of that shift, assisting them in resolving issues and bringing unresolved concerns to the director.
  • Handle patient/family and personal grievances and document per agency policy, referring incidents to the Director.
  • Interface professionally with physicians, vendors, other agencies/facilities, and other departments within Hospice.
  • Participate in family conferences when appropriate.
  • Assist in evaluation of Hospice Residence staff as per policy.
  • Assist in evaluation and implementation of orientation program for new Hospice Residence employees.
Requirements
  • Candidates must have a NYS RN Licensure with 1-2 years of recent experience employed as an RN in a hospital setting, home care, or skilled nursing facility.
  • Graduate of an accredited school of Nursing and currently licensed to practice in New York State.
  • Applicant must demonstrate basic knowledge of nursing theory and practice, knowledge of appropriate technology, knowledge of practice standards for area of responsibility, sound clinical skills, sound standards of practice, and positive communication skills.
  • Previous hospice/home care experience preferred.
  • Hospice is an equal opportunity employer.

The Registered Nurse will work in a fast-paced environment, providing care to patients and their families in a compassionate and respectful manner. The ideal candidate will be able to work effectively in a team setting, communicate clearly and professionally, and maintain a high level of emotional intelligence.

The Registered Nurse will be required to lift, position, and/or transfer patients, as well as lift supplies and equipment. The ability to walk, stand, and move around the Hospice Residence is also required.
