Job Title: Registered Nurse - Case Manager

St Croix Hospice is seeking a skilled Registered Nurse to join our team as a Case Manager. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and coordinating services to meet the needs of our patients and their families.

Job Summary:

The Registered Nurse Case Manager will be responsible for planning, organizing, and directing hospice care for patients and their families. This includes conducting comprehensive assessments, developing and implementing individualized care plans, and coordinating services with other healthcare professionals.

Key Responsibilities:
  • Conduct initial and ongoing comprehensive assessments of patients and their families to determine hospice needs
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families
  • Coordinate services with other healthcare professionals, including physicians, social workers, and spiritual care specialists
  • Provide education and support to patients and their families on disease management, symptom control, and end-of-life care
  • Collaborate with the interdisciplinary team to develop and implement care plans that meet the unique needs of each patient
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Graduate of an accredited school of nursing
  • Current licensure in the state of Indiana
  • One year of recent acute care experience in an institutional setting, and one year of recent experience in home care/hospice preferred
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist
