Nurse Practitioner

We are seeking a compassionate and skilled Registered Nurse to join our team as an On-Call RN at St Croix Hospice. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in their homes and communities.
Job Summary:The On-Call Registered Nurse is responsible for organizing and directing hospice care, providing emotional support and guidance to patients and their families, and collaborating with the interdisciplinary team to ensure seamless care delivery. This role requires strong communication and problem-solving skills, as well as the ability to work independently and make sound decisions in a fast-paced environment.
Responsibilities:- Provide comprehensive assessments and develop individualized care plans for patients and their families.
- Collaborate with the interdisciplinary team to ensure continuity of care and address patient needs.
- Respond to after-hours calls and provide emergency care as needed.
- Document patient interactions and care provided in a timely and accurate manner.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Graduate of an accredited school of nursing.
- Current licensure in the state and CPR certification.
- One (1) + years of recent acute care experience in an institutional setting, and one (1) + years of recent experience in home care/hospice.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist.
