Job Title: Nutritionist

Community Health of South Dade, Inc. is seeking a highly skilled Nutritionist to join our team.

Job Summary:

The Nutritionist will be responsible for coordinating, implementing, and evaluating Nutrition Program Activities for our various CHI sites. This includes providing in-depth counseling to individual patients, conducting regularly scheduled nutrition classes, and assisting with diabetic, weight reduction, hypertension, well-child care, and prenatal classes.

Responsibilities:
  • Coordinate with Providers to establish dietary treatment plans
  • Provide nutrition counseling and education to patients and their families
  • Assist with the development of departmental Policies and Procedures
  • Review and revise nutrition program Policies and Procedures
  • Interpret Nutrition Policies and Procedures for unit personnel, patients, and their families
  • Deliver and document patient care according to established Policies and Procedures
  • Record pertinent findings and actions taken in patient medical records
  • Provide professional growth and development opportunities for self and staff
  • Participate in continuing education, in-service training, and Performance Improvement Program activities
  • Maintain current CPR certification and professional registration
  • Organize and conduct nutrition education programs for staff
  • Develop and compile nutrition education material for staff and patients
  • Perform clinical and administrative duties in a professional manner
  • Maintain confidentiality regarding patient information
  • Recognize and respect patient rights and responsibilities
  • Provide leadership and complete assignments on time
  • Observe dress code and wear identification badge
  • Does not abuse personal days
  • Maintain open communications with other departments, administrative staff, and community agencies and organizations
  • Cooperate and integrate other disciplines in education programs developed
  • Participate in community programs, as assigned
  • Coordinate, evaluate, and implement Dietetic Services Standards
  • Provide technical assistance to the Cafeteria to ensure adequate nutritional care for CSU clients
  • Evaluate regular and modified menus for CSU clients
  • Maintain 80% productivity monthly
  • Reports to work on time and ready to work with minimal absenteeism
  • Completes B&E (billing and encounter) forms within the same day of intervention
  • Provides accurate and timely documentation in patient charts within the same day of intervention
  • Adheres to Confidentiality Policies and Procedures / HIPPA Regulations
