Role Summary
The Youth Services Placement Coordinator plays a crucial role in evaluating referrals and recommending suitable services tailored to the needs of the youth and families served by the organization. - Commit to three 12-hour shifts per week.
- Oversee placement procedures and manage interagency communication and case file documentation.
- Conduct initial assessments of referrals and, when necessary, perform on-site evaluations with the youth and their families.
- Recommend appropriate services based on assessment findings, ensuring the least restrictive environment for the youth.
- Facilitate communication with referring agencies and foster parents regarding the determination and negotiation of care levels based on assessments.
- Input referral information into agency systems, including SharePoint and Excel.
- Support the expansion of the referral network through approved strategies to enhance awareness of the organization's services.
- Prepare and distribute statistical reports to supervisors detailing service reception activities as required.
- Advocate for youth through individual and systemic advocacy to ensure effective service delivery.
- Adhere to agency protocols for emergency coverage during weekends and holidays.
- Engage effectively with agency personnel, foster parents, referring agencies, and the community.
- At least two years of experience in youth services, preferably with direct involvement in Human Services.
- A Bachelor's Degree in a social service or related discipline.
- Proficient in using computers, smartphones, tablets, and various software applications including word processing, spreadsheets, and databases.
- Exceptional customer service and communication abilities.
- A genuine commitment to advocating for at-risk youth.
- Valid driver's license.
- Reliable personal transportation.
- Clean driving record.
- Minimum automobile insurance coverage of $100,000/$300,000 for bodily injury liability.
