Key Responsibilities:
  • Develop and implement individualized physical therapy treatment plans to address patients' unique needs and goals.
  • Conduct thorough assessments and evaluations to identify areas for improvement and track progress.
  • Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure seamless care coordination.
  • Provide education and training to patients and their families on proper techniques, exercises, and self-care strategies.
  • Document patient progress, outcomes, and any changes to treatment plans in a timely and accurate manner.
Requirements:
  • Master's degree in Physical Therapy from an accredited program.
  • Current and unrestricted Physical Therapy license.
  • Minimum of one year of experience in home health or a related setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
