About the Role

The DMC Rehabilitation Institute of Michigan is a leading provider of rehabilitation medicine and research, with a strong focus on patient-centered care. We are currently seeking a skilled Healthcare Transition Specialist to join our team.

Job Summary

The successful candidate will be responsible for facilitating patient care along a continuum, ensuring seamless transitions between healthcare settings. This will involve effective resource coordination, timely communication, and collaboration with multidisciplinary teams.

Key Responsibilities

  • Care Coordination: Screen patients for factors affecting care progression and intervene as needed to promote timely throughput.
  • Transition Planning: Conduct assessments, stratify patients at risk for readmission, and develop plans to ensure safe and timely discharge.
  • Utilization Management: Balance clinical and financial requirements to advocate for patient needs while managing resources judiciously.
  • Documentation: Maintain accurate records in our Case Management system, including demographic and insurance information.
  • Compliance: Ensure adherence to federal, state, and local regulations, as well as accreditation standards impacting case management scope of services.

Requirements

  • Bachelor's degree in Nursing or other health-related field; equivalent combination of education and/or related experience acceptable.
  • TWO years of acute hospital patient care experience, with case management experience preferred.
  • Ability to complete Tenet's InterQual education course within 30 days of hire and maintain a score of 85 or better.

  • A competitive salary range: $65,000 - $80,000 per year, based on experience.
