Job Summary

We are seeking a highly motivated and experienced RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in providing high-quality patient care and coordinating services to ensure our patients receive the best possible care.

Key Responsibilities
  • Conduct thorough initial assessments of patients and develop comprehensive care plans to meet their unique needs.
  • Provide hands-on nursing care and education to patients and their families, promoting independence and self-care.
  • Maintain open and effective communication with patients, families, and the healthcare team to ensure seamless care coordination.
  • Manage patient care coordination, including scheduling appointments, arranging transportation, and coordinating with other healthcare providers.
  • Participate in interdisciplinary team meetings to ensure comprehensive care planning and coordination.
Requirements
  • Licensed as a Registered Nurse (RN) in the state of employment.
  • Proficiency in Electronic Medical Records (EMR) software.
  • Excellent planning, organizing, and communication skills.
  • Ability to work autonomously and make sound decisions in a fast-paced environment.
  • Valid Driver's License and reliable vehicle with current insurance.
