Job Summary

We are seeking an experienced Advanced Practice Nurse to join our team at Mass General Hospital. The successful candidate will work in a dynamic and innovative practice, providing timely and complete physical exams, chronic disease management, preventive healthcare, and urgent care to a designated group of primary care patients.

Responsibilities

  • Collaborate with physician partners, primary nurses, medical assistants, and support staff to deliver expert team-based primary care.
  • Perform and record patient history and physical exams, and discuss findings with physicians when appropriate.
  • Apply advanced nursing knowledge to identify, prevent, or solve complex and recurring patient care problems.
  • Follow and manage patients in collaboration with physicians, enhancing continuity of care.
  • Perform medically delegated aspects of patient care as agreed upon by nursing and medical administrators according to practice guidelines.
  • Maintain and practice in accordance with practitioner guidelines per state and hospital regulations.
  • Monitor patient outcomes in accordance with defined standards of patient care.
  • Serve as a resource to staff in areas of expertise.
  • Participate in the development of formal and informal educational programs for staff within areas of expertise.
  • Evaluate own practice and program efficiency.
  • Identify pertinent nursing and health delivery problems for investigation.
  • Evaluate current research findings and deliver evidence-based care.
  • Responsible for own professional growth and maintenance of required credentials.
  • Maintain standards and educational requirements specific to the position and licensure.
  • Identify own learning needs and goals.
  • Maintain and update clinical knowledge and skills based on current healthcare practice/research.
  • Identify system-wide factors contributing to undesirable outcomes, such as complications.

Requirements

  • 3 years of experience as an NP in primary care (preferred)
  • MSN required
  • Current license to practice as an NP in MA required
