Job Title: Ambulatory Care Nurse

We are seeking a skilled Ambulatory Care Nurse to join our team at Mass General Brigham. As an Ambulatory Care Nurse, you will be responsible for providing high-quality patient care in an outpatient setting.

Job Summary:

The Ambulatory Care Nurse will work collaboratively with a multidisciplinary team to provide comprehensive care to patients with various medical conditions. This includes assessing patients, developing and implementing individualized care plans, and educating patients and families on disease management and self-care.

Responsibilities:
  • Provide direct patient care in an ambulatory setting, including assessment, planning, implementation, and evaluation of patient care.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Conduct thorough assessments of patients, including medical history, physical examination, and laboratory results.
  • Collaborate with the healthcare team to develop and implement treatment plans.
  • Provide education and support to patients and families on disease management and self-care.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
  • Graduate of an approved school of nursing with current registration in Massachusetts.
  • Minimum of 1 year of clinical nursing experience in an ambulatory clinic/practice setting.
  • Experience with telephone triage and telephone-based clinical assessment techniques.
  • Experience providing in-person triage and direct patient care for acute and chronic conditions in an ambulatory outpatient setting.
  • Experience performing point of care testing and ambulatory clinic procedures.
Preferred Qualifications:
  • Demonstrated evidence of exemplary nursing practice and experience in multiple ambulatory specialties.
  • Case Management experience.
  • Experience with team-based care.
