Join Our Team as a MDS Coordinator

The Heights Post Acute is a leading provider of post-acute care services, and we are seeking a highly skilled MDS Coordinator to join our team. As a MDS Coordinator, you will play a critical role in ensuring the accuracy and completeness of resident care documentation, while maintaining confidentiality and adhering to regulatory requirements.

Key Responsibilities:
  • Collect and review MDS documents to ensure accuracy and completeness
  • Compare data entered against original MDS documents and make corrections as needed
  • File MDS documents in resident charts and maintain confidentiality of patient care information

Requirements:
  • Current LPN or RN license in good standing in CO
