Key Responsibilities- Develop and maintain relationships with healthcare professionals, referral sources, and patients to promote our hospice services.
- Provide education and training on hospice care to healthcare professionals, patients, and families.
- Conduct thorough assessments of patients' needs and develop individualized care plans to ensure optimal outcomes.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
- RN license in good standing in the state(s) of employment.
- Minimum of 2 years of experience in hospice, home health, or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
