Sutter Health is seeking a highly skilled Clinical Pharmacist to join our Oncology team. As a key member of our healthcare team, you will work closely with physicians and other healthcare providers to ensure optimal medication therapy for patients with cancer.
Key Responsibilities- Conduct patient interviews and counseling to optimize medication use and minimize adverse reactions
- Review medical records and medication profiles to identify potential medication-related problems
- Consult with physicians and other healthcare providers to develop and implement individualized medication plans
- Provide education and training to patients, families, and healthcare staff on medication use and management
- Monitor and evaluate medication use and outcomes to identify areas for improvement
- Doctorate in Pharmacy from an accredited institution
- 5 years of recent experience in clinical pharmacy practice, preferably in oncology
- Current licensure as a Pharmacist in the state of California
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
